Synopsis:
Golden
Rule Plan 100
This
plan covers 100% of covered expenses after the calendar year deductible,
including doctor office visit fees, prescription drugs, diagnostic
testing, and hospitalization and surgery, up to $3,000,000 per
covered person lifetime maximum. There are no copays for
doctor visits, but once the calendar year deductible is met the
insurance will pay 100% of covered expenses.
Golden
Rule Saver 80
This
is the least expensive Golden Rule plan available. It is
good for hospitalization and surgery, but does not cover most
outpatient expenses, including doctor office visits, diagnostic
testing, and prescription drugs. After the calendar year
deductible this plan pays 80% of the first $15,000 in covered
expenses, then 100% up to $3,000,000 per covered person lifetime
maximum.
Golden
Rule Copay Select
This
plan covers 80% of the first $10,000 of covered expenses after
the calendar year deductible, including doctor office visit fees,
prescription drugs, diagnostic testing, and hospitalization and
surgery, up to $3,000,000 per covered person lifetime maximum.
In-Network doctor visits are subject to a $35 copayment for history
and exam.
Golden
Rule Copay Saver
This
plan covers 80% of the first $15,000 covered expenses after the
calendar year deductible, including hospitalization and surgery,
up to $3,000,000 per covered person lifetime maximum. Doctor
visits are subject to a $35 copay for your first two visits per
person per year, after which doctor visits are not covered.
Outpatient prescriptions drugs are $15 copay for generic and name-brand
are not covered.

These
plans are available only to members of the Federation of American
Consumers and Travelers (FACT), an independent consumer organization.
If you are not already a member of FACT, you must join in order
to be eligible for these plans. Through a special agreement
between FACT and Golden Rule Insurance Company, you can enroll
in the association on their web site. You will fill out
the FACT enrollment form prior to making application to Golden
Rule for health insurance. For more information on the benefits
of FACT membership, visit www.usafact.org.
Estimated Monthly Premium does not include the mandatory $3 per
month dues for FACT membership. FACT membership is not required
in all states; refer to the product brochure for more information.

Rates depend on gender, family size, smoking status, zip code,

The
premium can be paid via quarterly billing, or a monthly bank draft.
The bank draft will occur on the premium due date each month.
The initial premium can be paid with a check, credit card, or
bank draft. This payment will be processed immediately upon
receipt.

Golden Rule utilizes the UnitedHealthOne Choice Plus network, one of
the largest and most well respected networks of doctors and medical facilities
across the nation. To find out which network is available to you in your

Underwriting:
UnitedHealthOne/Golden
Rule has one of the fastest underwriting departments in the industry.
Most applications are approved in less than a week. However, it could
take longer if medical records are requested, so it is best to allow 4 weeks
or more if possible. Certain conditions may be excluded with a temporary
rider (usually for one to two years), or an indefinite rider. Golden
Rule Insurance Company will typically call you on the phone to verify information
on your application. It
is standard practice to request a Blood Pressure Inquiry if you are currently
being treated for or have recently discontinued treatment for high blood pressure.

Effective
dates:
For
injuries, the effective date for a mailed application will
be the later of: (a) the requested effective date, if any, shown
on the application; or (b) the date upon which the original application
is actually received by Golden Rule.
For
an application sent by any electronic method, the effective date
for injuries will be the later of: (a) the requested effective
date, if any, shown on the application; or (b) the day after the
date upon which the application is actually received by Golden
Rule.
The
effective date for illnesses will be the same as for injuries
if you are replacing prior coverage within 62 days of application
for this coverage and disclose replacement information on the
initial application for insurance. If replacement information
is not disclosed on the initial application for insurance, the
effective date for illnesses will be the 15th day after the effective
date for injuries. Illnesses that begin prior to that 15th
day will be treated as a preexisting condition and will not be
covered until the individual has been a covered person for 12
months.
